Multiple key press required to enter punctuation?

Hello,

I am using the us keyboard layout and can not figure out why I
have to press punctuation keys twice to get the character.
The following keys are affected:

0x60 `
0x7E ~
0x5E ^
0x22 "
0x27 '

I am guessing it is a key map issue but I am stumped.
